According to PACER, in or around 2013, Elevate Recoveries, LLC sent a letter to a Florida consumer regarding a medical debt dating from 2011. She disputed it, saying that it would have been covered by her insurance. The agency allegedly forwarded a validation package that did not specify the amount owed, while the consumer had records stating that she owed nothing.
